This notice informs you that your IRS Form 1095-C for the 2025 tax year is available upon request. Due to recent changes in federal law, Parker Subaru is no longer required to automatically send these forms to all eligible individuals.

To request a copy of your form, please contact us using one of the methods below. We will provide your form within 30 days of receiving your request.
